Perbandingan Konsumsi Oksigen Maksimum pada Pasien Penyakit Jantung Hipertensi dan Pasien Hipertensi di UPT Puskesmas Cipamokolan

Abstract


Abstract. Hypertension is a disease that can affect several target organs, such as peripheral arteries and  heart. chronic hypertension that causes hypertensive heart disease that begins with left ventr hypertro n causes func capacity decreased. Functional Capacity Assessment is the easiest to do and has an high accuration with a 6 minute walk test. The study aims to determine the study of maximum oxygen consumption in hypertensive heart disease patients and hypertension patients in UPT Cipamokolan Health Center. This study was an observational analytic using the cross-sectional method. The subjects were hypertensive heart disease patients and hypertension patients at the Cipamokolan Health Center UPT. The study was done by taking secondary data from the patient's medical records and primary data by doing a 6 minute walk test for 32 patients and converted to the maximum oxygen consumption formula. The average amount of maximum oxygen consumption in hypertensive patients is higher at 14.84 ml / kg / min compared to the average maximum oxygen consumption in patients with hypertensive heart disease of 13.24 ml / kg / min. Statistical test results used an Independent t-test at a 95% confidence level for hypertension with the significance of hypertensive patients and hypertensive patients with a p value of 0.001 (p value of 0.05). this is caused by hypertrophy of the left ventricle then functional capacity and cardiac output decreased.

Abstrak. Hipertensi merupakan penyakit yang dapat mengenai beberapa organ target, seperti arteri perifer dan jantung. Hipertensi yang kronis menyebabkan penyakit jantung hipertensi yang di awali hipertrofi ventrikel kiri dan menyebabkan penurunan dari kapasitas fungsional. Penilaian kapasitas fungsional yang paling mudah dilakukan dan memiliki akurasi cukup tinggi yaitu dengan uji jalan 6 menit. Tujuan penelitian ini adalah mengetahui perbandingan konsumsi oksigen maksimum pada pasien penyakit jantung hipertensi dan hipertensi di UPT Puskesmas Cipamokolan. Penelitian ini adalah observasi analitik menggunakan metode potonglintang. Subjek penelitian adalah pasien penyakit jantung hipertensi dan pasien hipertensi di UPT Puskesmas Cipamokolan. Penelitian di lakukan dengan cara mengambil data sekunder dari hasil rekam medis pasien dan data primer dengan cara melakukan uji jalan 6 menit kepada 32 pasien dan dikonversikan ke rumus konsumsi oksigen maksimum. Hasilnya di dapatkan rata – rata  nilai konsumsi oksigen maksimum pada pasien hipertensi lebih tinggi yaitu sebesar 14,84 ml/kg/min dibandingkan dengan  rata – rata konsumsi oksigen maksimum pada pasien penyakit jantung hipertensi yaitu sebesar 13,24 ml/kg/min. Hasil uji statistik menggunakan Independent t test pada derajat kepercayaan 95% menunjukkan bahwa terdapat perbandingan konsumsi oksigen maksimum yang signifikan pada pasien penyakit jantung hipertensi dan pasien hipertensi dengan nilai p sebesar 0,001 (nilai p≤0,05). Hal ini disebabkan terdapat hipertrofi dan dilatasi ventrikel kiri pada penyakit jantung hipertensi sehingga kapasitas fungsional dan curah jantung menurun.
