Pengaruh Indeks Massa Tubuh Terhadap Fungsi Ventrikel Kiri Berdasarkan Pemeriksaan Ekokardiografi Pada Kelompok Obesitas Dewasa Muda

Abstract


Obesitas menjadi masalah kesehatan yang terus berkembang. Pengaruh perbedaan ras yang berhubungan dengan indeks massa tubuh dan dapat berpengaruh terhadap fungsi jantung khususnya ventrikel kiri. Penelitian ini bertujuan untuk membuktikan apakah terdapat hubungan antara indeks massa tubuh dan fungsi ventrikel kiri pada kelompok obesitas dewasa muda. Penelitian dilakukan dengan desain analitik observasional korelasi dengan metode potong lintang, teknik pengambilan sampel adalah consecutive sampling dengan mengambil 29 data rekam medis berdasarkan pemeriksaan ekokardiografi pada subjek kelompok obesitas usia dewasa muda di Instalasi Pelayanan Jantung RSUP Dr. Hasan Sadikin Bandung. Data dikelompokan berdasarkan karakteristik antropometri yang terdiri dari jenis kelamin, usia, tinggi badan, berat badan dan indeks massa tubuh dan karakteristik ekokardiografi ventrikel kiri yang dibagi ke dalam parameter dimensi ruang jantung dan fungsi sistolik (LVEDs, LVEDd dan FEV) juga parameter diastolik (E/A dan DT).   Berdasarkan hasil analisis uji chi square diperoleh hasil  terdapat hubungan antara indeks massa tubuh dan fungsi diastolik ventrikel kiri (E/A) dengan nilai P= 0.04 . Namun tidak terdapat hubungan dengan parameter lainnya (LVEDs, LVEDd, FEV dan DT). Dengan demikian dapat disimpulkan indeks massa tubuh memiliki korelasi positif dengan fungsi diastolik ventrikel kiri pada kelompok obesitas dewasa muda namun tidak untuk dimensi ruang jantung dan fungsi sistolik.

Obesity is a growing health problem. Effect of racial differences related to body mass index and also can affect heart function, especially the left ventricle. This study aims to prove whether there is a relationship between body mass index and left ventricular function in the obese young adult group. The study was conducted by observational analytic design correlation with cross-sectional methods, the sampling technique was consecutive sampling by taking 29 medical record data based on echocardiographic examination in obese young adults in the Heart Service Installation Hospital Dr. Hasan Sadikin Bandung. Data are grouped based on anthropometric characteristics consisting of gender, age, height, weight and body mass index and characteristics of left ventricular echocardiography divided into parameters of cardiac space dimensions and systolic function (LVEDs, LVEDd and FEV) as well as diastolic parameters (E / A and DT). Based on the results of the chi square test the results obtained there is a relationship between body mass index and left ventricular diastolic function (E / A) with P value = 0.04. But there is no relationship with other parameters (LVEDs, LVEDd, FEV and DT). Thus it can be concluded that body mass index has a positive correlation with left ventricular diastolic function in the obese group of young adults but not for the dimensions of the heart space and systolic function.
