Hubungan Indeks Massa Tubuh dan Lingkar Leher dengan Risiko Obstructive Sleep Apnea pada Pasien Parkinson Di RSAU Salamun Kota Bandung

Abstract


Abstract. Obstructive sleep apnea (OSA) is a disease characterized by repeated episodes of upper airway obstruction which causes awakening from repeated sleep. The prevalence of OSA in the normal population in Jakarta is 49.5%. Predisposing factors for OSA include obesity, middle age, menopause in women, large neck circumference, craniofacial structural abnormalities, and upper airway structural abnormalities. One of risk factors of OSA is obesity with Body Mass Index (BMI) is more than 30kg/m. Neck circumference is another anthropometric measurement that can assess obesity and is included in the STOP-BANG Questionnaire with a neck circumference> 40cm. The aim of this study is to analysed the association of BMI and neck circumference with OSA risk in Parkinson's patients. The method of this study used observational analytic with cross-sectional design conducted with the subject of research as many as 50 Parkinson patients in nerve polyclinic RSAU Salamun on March - July 2018. Measurements of body weight and height for BMI and neck circumference were measured, then determined the degree of risk OSA is assessed using the STOP-BANG score consisting of eight questions. The results showed a BMI value of OSA 0.032 and a neck circumference value of OSA 0.000 had a significant relationship (p <0.05). In conclusion, there is a significant relationship between BMI and neck circumference with the risk of OSA in Parkinson's patients. Suggestion for OSA examination and aiming for early detection in Parkinson's patients is recommended.

Abstrak. Obstructive sleep apnea (OSA) merupakan penyakit yang dikarakteristikan dengan adanya episode berulang dari obstruksi saluran nafas atas yang menyebabkan terbangun dari tidur secara berulang. Prevalensi OSA pada populasi normal di Jakarta sebesar 49,5%. Faktor predisposisi terjadinya OSA diantaranya obesitas, usia paruh baya, menopause pada wanita, lingkar leher yang besar, abnormalitas struktur kraniofasial, serta abnormalitas struktur saluran napas atas. Salah satu faktor risiko dari OSA adalah obesitas dengan Indeks Massa Tubuh (IMT) lebih dari 30kg/m. Lingkar leher (neck circumference) merupakan pengukuran antropometri lain yang dapat menilai obesitas dan termasuk pada STOP-BANG Questionnaire dengan nilai lingkar leher >40cm. Tujuan penelitian ini menganalisis hubungan IMT dan lingkar leher dengan risiko OSA pada pasien Parkinson. Metode penelitian ini menggunakan analitik observasional dengan rancangan potong lintang yang dilakukan dengan subjek penelitian sebanyak 50 orang pasien Parkinson di poli saraf RSAU Salamun periode maret – juli 2018. Dilakukan pengukuran berat badan dan tinggi badan untuk pengukuran IMT dan lingkar leher, lalu ditentukan derajat risiko kejadian OSA diinilai menggunakan skor STOP-BANG yang terdiri dari delapan pertanyaan. Hasil penelitian menunjukan nilai IMT terhadap OSA 0,032 dan nilai lingkar leher terhadap OSA 0,000 terdapat hubungan bermakna (p< 0,05). Simpulannya, terdapat hubungan bermakna antara IMT dan lingkar leher dengan risiko terjadi OSA pada pasien Parkinson. Disarankan dilakukan sosialisasi dan pemeriksaan OSA bertujuan untuk deteksi dini pada pasien Parkinson.
