Analisis Faktor Usia, Jenis Pekerjaan dan Status Paritas dengan Derajat Hemoroid Internal

Abstract


Abstract.Internal Hemorrhoid is a result from dilation of the superior hemorrhoidal veins. Although Internal hemorrhoids is not always a serious health problem and life-threatening, but it can causes discomfort and interfere activity of the sufferer. Few literatures explain that Internal hemorrhoids is influenced by many factors including age, occupation, parity status. The aim of this research was to determine the relationship between age, occupation and parity status with the grade of internal hemorrhoids. This research was an observational study with cross sectional method. Respondent who came to Departement of Surgical in Al-Ihsan General Hospital from March to May 2018 has been asking to fill the questionnaires. The number of sample were 63 persons. From 63 persons who has diagnosed with internal hemorrhoids, mostly aged > 45 years (73.02%). For the type of the occupation most workers do not require prolonged seats (76.19%), and lifting heavy loads (95.24%). For the number of children most of them have 3 children (39.68%). Data analysis using fisher's exact method, the result showed significant. There was a significant relationship between age, parity status and grade of internal hemorrhoid (p value = 0.00). Meanwhile, work requiring prolonged sitting and heavy lifting did not have a significant relationship with grade of internal hemorrhoids (each p value= 0.83 and 0.92). Significant relationship caused by the destruction of stromal connective tissue resulting in prolapse of hemorrhoidal tissue it can be caused by changes in age-related collagen formation, and hemorrhoids can occur due to excessive straining during labor, allowing the vein in the anorectal region are widen. Non-significant relationship caused by mostly women are rare do heavy activities and sit longer if compared with men.

Abstrak. Hemoroid internal adalah pelebaran vena hemoroid superior. Hemoroid internal tidak selalu menjadi masalah kesehatan yang serius dan mengancam nyawa, namun penyakit ini bisa menyebabkan ketidaknyamanan dan menganggu aktivitas penderitanya. Beberapa literatur menyatakan hemoroid internal dipengaruhi oleh banyak faktor diantaranya usia, jenis pekerjaan, status paritas. Tujuan penelitian ini adalah untuk mengetahui hubungan usia, jenis pekerjaan dan status paritas dengan derajat hemoroid internal. Penelitian ini menggunakan analisis observasional dengan rancangan cross-sectional. Responden wanita yang datang ke RSUD Al-Ihsan pada periode Maret-Mei 2018 diminta mengisi kuesioner. Sampel pada penelitian ini adalah 63 orang. Dari 63 pasien yang diteliti, sebagian besar berusia >45 tahun (73.02%). Untuk jenis pekerjaan yang dilakukan sebagian besar pekerjaan tidak mengharuskan duduk lama (76.19%), dan mengangkat beban berat (95.24%). Untuk jumlah anak yang dimiliki sebagian besar memiliki 3 anak (39.68%). Analisis data menggunakan metode fisher’s exact, hasilnya menunjukkan signifikan. terdapat hubungan yang bermakna  antara usia, status paritas dengan derajat hemoroid internal (p = 0.00). pekerjaan yang mengharuskan duduk lama dan mengangkat beban berat tidak memiliki hubungan yang bermakna dengan derajat hemoroid internal (p=0,83 dan p=0,92). Hubungan yang bermakna disebabkan oleh penghancuran jaringan ikat stroma yang mengakibatkan prolaps jaringan hemoroid dianggap disebabkan oleh perubahan pada pembentukan kolagen terkait usia, dan hemoroid bisa terjadi akibat mengedan terlalu berlebihan saat persalinan berlangsung, sehingga vena di daerah anorektal semakin melebar. Hubungan yang tidak bermakna disebabkan karena wanita jarang melakukan aktivitas yang berat dan duduk lama jika dibandingkan dengan pria.
