Hubungan Lama Pemasangan Foley Kateter Uretra dengan Kejadian Infeksi Saluran Kemih pada Pasien Benign Prostatic Hyperplasia di RSUD Al-Ihsan Periode Januari sampai Desember 2017

Abstract


Abstract.Benign Prostatic Hyperplasia (BPH) is a benign tumor that occurs in men, and its incidence is age-related, which is a diagnosis by histopathology. BPH patients experience symptoms of Lower Urinary Tract Symtoms (LUTS) such as difficulty urinating, micturition is painful, an average of 32 months before the Acute Urinary Retention (AUR) occurs, when the patient experiences AUR the patient is really in pain and unable to remove the urine so the patient must Foley urethral catheter is inserted to remove retained urine. In the installation of a foley catheter that is too long or> 2 weeks will cause bacteremia in patients so that patients are at risk of developing urinary tract infections. This study used a paired bivariate comparative analytical retrospective method with a cross-sectional approach. Data obtained through 48 medical records data of BPH patients with criteria of patients who installed foley urethra catheter and leukocyte laboratory results before surgery, data collection using Simple Random Sampling method. The results of this study showed the installation of foley catheter based on <2 weeks as much as (66.7%) while the installation> 2 weeks as much (33.3%), based on laboratory results of leukocyte installation> 2 weeks of infection (75%) and not infection (25%), installation <2 weeks of infection (6.4%) and not infection (93.8). The research data was analyzed using Chi square test. The results showed p value = 0.001 it can be concluded that there was a correlation between the length of catheter insertion and the incidence of urinary tract infection (p value <0.05).

Abstrak.Benign Prostatic Hyperplasia adalah tumor jinak yang terjadi pada pria, dan kejadiannya terkait dengan usia, yang diagnosis dengan histopatologi. Pasien BPH mengalami gejala Lower Urinary Tract Symtoms (LUTS) seperti sulit berkemih, berkemih terasa nyeri, rata-rata 32 bulan sebelum terjadi Acute Urinary Retention (AUR), saat pasien mengalami AUR pasien benar-benar kesakitan dan tidak bisa mengeluarkan urinnya sehingga pasien harus di lakukan pemasangan foley kateter uretra untuk mengeluarkan urin yang tertahan. Pada pemasangan foley kateter yang terlalu lama atau >2 minggu akan menyebabkan bakteremia pada pasien sehingga pasien beresiko mengalami Infeksi Saluran Kemih. Penelitian ini menggunakan metode retrospektif analik komparatif bivariat tidak berpasangan dengan pendekatan cross-sectional. Data yang didapat melalui rekam medis 48 data pasien BPH dengan kriteria pasien yang di pasang foley kateter urethra dan hasil laboratorium leukosit sebelum operasi, pengambilan data dengan metode Simple Random Sampling. Hasil Penelitian ini menunjukkan pemasangan foley kateter berdasarkan waktu <2minggu sebanyak (66,7%) sedangkan pemasangan >2minggu sebanyak (33,3%), berdasarkan hasil laboratorium leukosit pemasangan >2minggu infeksi (75%) dan tidak infeksi (25%), pemasangan <2minggu infeksi (6,4%) dan tidak infeksi (93,8). Data penelitian di analisis menggunakan uji Chi square. Hasil penelitian menunjukkan Nilai p  = 0,001 dapat disimpulkan terdapatnya hubungan lama pemasangan kateter dengan kejadian Infeksi Saluran Kemih ( nilai p <0,05).
