Pengaruh Status Gizi terhadap Luaran Klinis Pasien Stroke Iskemik di Rsau Dr. M. Salamun Kota Bandung Tahun 2019 – 2020

Abstract


Abstract. Stroke is a non-communicable disease with high morbidity and mortality, especially in developing countries. Morbidity in stroke patients can cause neurological deficits as a decrease in the body's functional status and disability which can also cause death. This study aims to determine the effect of nutritional status on the clinical outcome of stroke patients in RSAU Dr. M. Salamun, Bandung. The study design was an observational analyze with cross sectional design. Data were taken using purposive sampling technique from January 2019 – December 2020 with a total number of subject is 108 people. The subjects nutritional status was assessed using BMI and clinical outcomes measured by using the mRS (modified rankin scale). The data obtained were carried out by statistical tests using the kendall corellation test. The results showed that the majority of patients nutritional status were normal (40,7%) with good clinical outcome (57,4%). The results of the analysis showed that there was an impact of nutritional status on clinical outcome of ischemic stroke in RSAU Dr. M. Salamun (p < 0.05). The conclusions of the research results indicate that there is an impact of nutritional status on clinical outcome. This occurs because there is influence of normal nutritional status that has good metabolic reserve and body component that is neuroprotective to prevent hypercatabolic process which can improve ischemic stroke outcome.

Abstrak. Stroke merupakan salah satu penyakit kardiovaskular yang paling sering terjadi, dengan morbiditas dan mortalitas yang tinggi terutama di negara berkembang. Morbiditas menyebabkan terjadinya defisit neurologis berupa adanya penurunan pada status fungsional tubuh yang bersifat membaik, menetap, bahkan memburuk hingga dapat menyebabkan kematian. Status gizi merupakan salah satu faktor yang dapat memengaruhi luaran klinis pasien stroke. Penelitian ini bertujuan untuk mengetahui pengaruh status gizi terhadap luaran klinis pasien stroke iskemik di RSAU Dr. M. Salamun Kota Bandung. Desain penelitian bersifat analitik observasional dengan metode rancangan potong lintang. Data diambil dengan menggunakan teknik purposive sampling pada periode Januari 2019 - Desember 2020 dengan jumlah subjek penelitian sebanyak 108 orang. Penelitian dilakukan dengan cara menilai status gizi dengan menggunakan BMI dan luaran klinis menggunakan mRS (Modified Rankin Scale). Analisis data dilakukan menggunakan uji korelasi kendal. Hasil penelitian menunjukkan sebagian besar pasien memiliki status gizi normal (40,7%) dan luaran klinis baik (57,4%) serta didapatkan adanya pengaruh status gizi terhadap luaran klinis stroke iskemik di RSAU Dr. M. Salamun (p<0,05). Simpulan hasil penelitian menunjukkan terdapat pengaruh antara status gizi terhadap luaran klinis pasien stroke iskemik di RSAU Dr. M. Salamun. Hal ini dapat terjadi karena pasien dengan status gizi normal memiliki cadangan metabolisme dan komponen tubuh yang baik dan bersifat neuroprotektif untuk mencegah proses hiperkatabolisme sehingga pasien stroke iskemik memiliki luaran klinis yang baik.
