Scoping Review: Hubungan Kepatuhan Pengobatan dengan Kadar Hba1c pada Pasien Diabetes Melitus Tipe 2

Abstract


Abstract. Diabetes mellitus globally is the leading cause of morbidity and mortality. This is related to the high complication of type 2 Diabetes Mellitus. The occurrence of complications is closely related to poor blood sugar control. Complications of type 2 diabetes mellitus can be controlled by controlling blood sugar. Medication adherence related to controlling HbA1c levels is important to achieve treatment goals and is effective in the process of preventing complications of type 2 Diabetes Mellitus. This study aims to determine the relationship between medication adherence and HbA1c levels. The method used in this study is Scoping Review, with research articles from international journals that meet the inclusion and exclusion criteria. This article was then selected using the PRISMA diagram by considering Population (Type 2 Diabetes Mellitus), Intervention (treatment adherence), Comparison (non-adherence medication), Outcome (HbA1c levels), Study (Observational). There are 10 articles that can be used for this research. The results of 9 articles showed that patients with type 2 diabetes mellitus with high medication adherence had low HbA1c values and there was a significant relationship between medication adherence and HbA1c levels. One research article states that there is no significant relationship between medication adherence with HbA1c, because medication adherence alone is not sufficient to control glycemic status, there are other factors, namely proper pharmacotherapy also affects glycemic status.

Abstrak. Diabetes melitus secara global dianggap sebagai penyebab utama morbiditas dan mortalitas. Hal ini berkaitan dengan tingginya komplikasi penyakit diabetes melitus tipe 2. Terjadinya komplikasi berhubungan erat dengan kontrol gula darah yang buruk. Komplikasi diabetes melitus tipe 2 dapat dikendalikan dengan mengontrol gula darah. Kepatuhan pengobatan terkait dengan pengendalian kadar HbA1c merupakan hal penting untuk mencapai sasaran pengobatan dan efektif dalam proses pencegahan komplikasi penyakit diabetes melitus tipe 2. Penelitian ini bertujuan untuk mengetahui hubungan kepatuhan pengobatan dengan kadar HbA1c. Metode yang digunakan dalam penelitian ini adalah scoping review, dengan bahan penelitian artikel dari jurnal internasional yang memenuhi kriteria inklusi dan eksklusi. Artikel ini kemudian diseleksi menggunakan diagram PRISMA dengan mempertimbangkan Population (Diabetes Melitus tipe 2), Intervention (kepatuhan pengobatan), Comparison (ketidakpatuhan terhadap pengobatan), Outcome (kadar HbA1c), Study (Observational). Didapatkan 10 artikel yang dapat digunakan untuk penelitian ini. Hasilnya 9 artikel menunjukan bahwa pasien diabetes melitus tipe 2 dengan kepatuhan pengobatan tinggi memiliki nilai HbA1c yang rendah dan terdapat hubungan yang signifikan antara kepatuhan pengobatan dengan kadar HbA1c. Satu artikel penelitian menyatakan bahwa tidak terdapat hubungan signifikan antara kepatuhan pengobatan dengan kadar HbA1c, karena kepatuhan pengobatan saja tidak cukup untuk mengontrol status glikemik, namun ada faktor lain yaitu farmakoterapi yang tepat.
