Hubungan antara Derajat Stenosis Arteri Koroner dan Kadar Troponin I pada Pasien Infark Miokard Akut di RSUP Dr. Hasan Sadikin Bandung Periode Januari 2018 sampai September 2019

Abstract


Abstract. Acute myocardial infarction (IMA) is mostly caused by rupture of atherosclerotic plaque causing myocardial necrosis. Ruptured plaque can cause blockage or coronary artery stenosis in one blood vessel (1 VD), two blood vessels (2 VD), three blood vessels (3 VD) or in the main left coronary artery. Troponin I is a biomarka detected in the blood and used to diagnose acute myocardial infarction. The purpose of this study was to determine the relationship between the degree of coronary artery stenosis and troponin I levels in patients with acute myocardial infarction at RSUP Dr. Hasan Sadikin Bandung from January 2018 to September 2019. The subjects were myocardial infarction patients who performed coronary angiography and include inclusion criteria. Data were taken using purposive sampling technique with 91 subjects. The research was observational analytic with cross sectional design. Data were analyzed by chi-square test using SPSS for windows ver. 24.0. The results showed no relationship between the degree of coronary artery stenosis and troponin I levels in patients with acute myocardial infarction with a value of p = 0.14 (p value <0.05). Based on the result of this study conclude that there is no significant relationship between the degree of coronary artery stenosis and troponin I levels in patients with acute myocardial infarction.

Abstrak. Infark miokard akut (IMA) sebagian besar disebabkan karena rupturnya plak aterosklerosis sehingga menyebabkan nekrosis miokardium. Rupturnya plak dapat menyebabkan sumbatan atau stenosis arteri koroner pada satu pembuluh darah (1VD), dua pembuluh darah (2 VD), tiga pembuluh darah (3 VD) atau pada left main coronary artery. Troponin I merupakan salah satu biomarka yang terdeteksi dalam darah dan digunakan untuk mendiagnosis infark miokard akut. Penelitian ini bertujuan untuk mengetahui hubungan antara derajat stenosis arteri koroner dan kadar troponin I pada pasien infark miokard akut di RSUP Dr. Hasan Sadikin Bandung periode Januari 2018 sampai September 2019. Subjek penelitian adalah pasien infark miokard akut yang melakukan angiografi koroner yang memenuhi kriteria inklusi. Data diambil menggunakan teknik purposive sampling dengan jumlah subjek 91 pasien. Metode penelitian ini adalah analitik observasional dengan desain potong lintang. Data dianalisis dengan uji chi-square menggunakan SPSS for windows ver. 24.0. Hasil penelitian menunjukkan tidak terdapat hubungan bermakna antara derajat stenosis arteri koroner dan kadar troponin I pada pasien infark miokard akut dengan nilai p= 0,14 (nilai p<0,05). Berdasarkan penelitian dapat disimpulkan bahwa tidak terdapat hubungan bermakna antara derajat stenosis arteri koroner dan kadar troponin I pada pasien infark miokard akut.
